Membership Supplies

These Membership Supplies Documents are Members Only Files. You will need your Member's Only username and password located on the back of your PTA membership card to view the files.

Membership Forms

We are phasing out the Membership Envelopes this year. Many local PTAs have asked us to provide a downloadable form as a more efficient option. This allows each local unit to easily print forms as you need more.
When you collect dues, make sure you calculate
$2.25 for National PTA + $1.75 for NCPTA + Local Dues
NOTE:  Remit both national and state
dues to the state office monthly, and we will submit National PTA dues for you. Please send any collected dues by the 15th of each month beginning July 15.

Membership Cards

National PTA provides membership cards to NCPTA. The NCPTA Office automatically sends membership cards to units in good standing in late July or early August to the school to the attention of the local unit president based on the previous year’s membership. The units that are not in good standing will receive their cards after the good standing requirements are met. Additional cards may be requested from the NCPTA Office after payment of dues for 80% of the cards previously issued is received.
Local units must enter their individual National/User ID number on each card. This number can be found on the Membership Card Accountability Sheet that is sent with the original allotment of membership cards. Membership cards should be given promptly to those joining since they are required for a person to be eligible to vote or hold office in a local unit. Please be aware that membership in one unit does not constitute membership and voting privileges in another unit.
Please make your members aware of the username and password on the back of the membership card.  This information can be used to access the Member's Only Section of the National PTA website (www.pta.org) and NCPTA website at www.ncpta.org .
Membership cards sent from the NCPTA Office must be accounted for, either by dues remitted to the State Office or by returning unused or voided cards to the NCPTA Office by June 15th

To receive membership cards, you must submit membership dues to the State PTA office (at least 80% of the cards previously issued) along with the form below.
