Chair Materials and Requirements

- Chair Checklist: Reflections Chairs- this form does not have to be submitted to the next level of judging. It is a tool to help you prepare your entries for the next level of judging
Participation Forms: A participation form is REQUIRED from ALL PTAs (Local units AND COUNCILS) participating in the PTA Reflections Program.
National PTA requires each state to track the number of PTAs that participated and the total number of works entered in the state’s Reflections Program. The participation forms make that possible and are vital to understanding how the Reflections Program shifts and grows each year.
Use the appropriate participation form below to record the number of entries judged by your PTA/Council as well as the number of entries forwarded to the next level of judging. Please provide all of the information requested, including contact information. Then, submit the form to the next level of judging along with winning artwork. Keep a copy of the form for your records. (Sample completed form is provided below)
